As to how he got in Gotham, I just went with "He's the god damn Batman". He's trained to infiltrate, sneak onto a ship that's headed to America, contact some friends, get a ride close to Gotham, slip into the city. They could have shown it but honestly the moving was already running really long as is and wouldn't needed showing. He's a ninja, he does what ninjas do.

Amazing movie, But Honestly, What happened to ALL the other Villains? I know I might have missed a movie or two, But Seriously.
Ra's al Ghul died in a train crash, Two Face died at the end of TDK, Jonathan Crane AKA Scarecrow made an apperance in TDKR as a judge, and Joker... well, according to the TDKR novel, nobody knows what happened to him. Either he is the only prisoner in Arkham or he escaped.
